Registration Form

Our registration process is easy!  Follow the steps below.  NOTE:  There is a $50 annual registration fee.  Discounts apply for returning families.

• Step 1:  Print and complete the Delaware Registration form or Maryland Registration form.   Registration fee is non-refundable.
• Step 2:  Have your child's pediatrician conduct a physical and complete a health appraisal form.  (Must be handed in within 30 days after school begins.)

• Step 3:  Send or drop off the following:  Registration form, Health Appraisal (if available), and Registration Fee to Bright Beginnings Preschool.
